Fall 2011 Dayspring Orientation and Back-to-School Information

Below are links to several documents connected with the start of the 2011-2012 school year at Dayspring Christian Academy (DCA). Feel free to download and fill-in these forms before Parent Orientation Night (Friday, August 19). Hardcopies will also be available that evening at the check-in tables. Forms not turned in at the Orientation Night should be turned in to the Dayspring main office as soon as possible.

- Parent Information Sheet [doc, pdf]. Parent contact information used for communication and emergency contacts.
- Medical Release and Annual Field Trip Form [doc, pdf]. Required for each student. Notarized signatures required. Notaries will be available at Parent Orientation Night. IDs are required for each parent signature. If one parent is unable to attend Orientation Night, be sure to bring along an ID corresponding to that parent's signature.
- Photograph Consent Form [doc, pdf]. For all students at DCA.
- Pick-Up Permission Form [doc, pdf]. For any student who will be picked up from school during the school year, i.e., all students except student drivers.
- (NEW) Senior Privileges Form [doc, pdf].
Reminder for New Students: the school needs a copy of your birth certificate and immunization record.
Reminder for Sixth Graders: the school needs written verification that you have had your TDAP vaccine. Information can be faxed from your doctor's office.

Parent Volunteer Opportunities DCA depends on the involvement of parents in all aspects of our mission. Needs and opportunities abound! Please complete this on-line survey to indicate ways you would like to contribute this year. (We ask that parents complete this survey each year because interests and availability often change from year to year.)
