• School Supply List: K-5th grades, 6th-12th grades

• Parent Information Sheet [doc, pdf]. Parent contact information used for communication and emergency contacts.

• Medical Release and Annual Field Trip Form [pdf]. Required for each student. Notarized signatures required. Notaries will be available at Parent Orientation Night. IDs are required for each parent signature. If one parent is unable to attend Orientation Night, be sure to bring along an ID corresponding to that parent's signature.

• Parent Volunteer Opportunities [doc, pdf]. Needs and opportunities for parent volunteers. Please read over and indicate your preferences on the last page.

• High School Student Driver Information [doc, pdf]. Required of all student drivers.

• Volunteer Driver Application Form [doc, pdf]. Required to drive for a Dayspring field trip.

• High School Retreat Information: August 26-28, Doe River Gorge, TN; $125/student; Summary and Doe River Form.
